Creating an On-line Survey/Questionnaire

If you wish to create an on-line survey or questionnaire, then we would strongly recommend that you start with Qualtrics. You must create an account linked to your University of Warwick ID (as instructed below) and you must not use a pre-existing personal Qualtrics account.

• The Qualtrics Research Suite is a user-friendly, feature-rich, web-based survey tool which allows users to build, distribute and analyse online surveys, collaborate in real-time and export data in multiple formats.
• WMG has a site licence for Qualtrics and it is WMG's preferred and supported survey software.

Accessing WMG's Qualtrics:

1. Go to the WMG Qualtrics site:
   https://warwickwmg.eu.qualtrics.comLink opens in a new windowLink opens in a new window
2. On your first visit you should choose the option "I don’t have a Qualtrics account"
   
3. Login with your IT username and password

This will set up your Qualtrics account connected to your University of Warwick IT account, and should be used to log into Qualtrics on all subsequent occasions.

Privacy Statement

Every questionnaire or survey must have a privacy statement that reflects the purpose of your form. Your statement must say something about what the data you're collecting will be used for, how long it will be retained, and who will have access to it. Please refer to the Ethics training you will have completed.
