We would like to ask you to provide information for an emergency contact. This would be someone we (IER Director of Graduate Studies and/or IER PhD Administrative Officer) can contact if your supervisors have not had any contact with you, you have missed a supervisory meeting and/or we have concerns about your well-being. We would be grateful if you could provide contact details for a family member or friend and give us permission to make contact if we have not had any communication from you and are concerned.

If you have more than one contact, please feel to submit multiple forms. We will ask you at the start of each academic year to update this information.

Please provide your emergency contact with information on how to contact IER in case you are not unable to do so. This could include contact details for your supervisors and/or the following:

The information collected will only be used if we (your supervisors, the IER DGS and/or the IER PhD Administrative Officer) have not had contact from you and we are concerned about your well-being.  The information will only be held during your registration period at the University of Warwick.
