Maternity Pay Form

Student Details

Any holder of a UKRI Studentship or University funded PGR Scholarship who is confirmed pregnant by a GP with form MATB1 (or equivalent) on which the expected week of childbirth is within the Studentship or Scholarship period is entitled to suspend their studies for up to 12 months as maternity leave. Adoption leave will be granted on the same basis as maternity leave.

Leave Period

Payments will be made as up to 26 weeks capped at the UKRI standard rate, then 13 weeks at the reduced SMP rate, then 13 weeks unpaid. This brings the total period of paid and unpaid leave to up to 52 weeks. In order to retain maternity leave payments, students must agree to return to their studies for a minimum of 3 months once their period of maternity leave has finished.

I confirm that I agree to repay any maternity pay stipend provided to me should I fail to return to my studies for a minimum of 3 months, or until I submit my thesis, once the period of maternity leave has finished. (required)
I understand that an application for maternity pay must be submitted at least two weeks in advance of when the leave is due to be taken. Maternity cannot be backdated (excluding exceptional circumstance i.e. premature birth of the child, that make this impossible). (required)
I have submitted a temporary withdrawal or authorised absence request with the same dates as this claim for maternity pay, including providing formal evidence (MATB1 or equivalent). (required)
