We are two PhD students and General practitioner trainees, Katrine Bjørnshave Bomholt and Mette Amalie Nebsbjerg, from the Research Unit for General Practice in Aarhus, Denmark. We are a part of the group “Acute Primary Care Research” our supervisors are Morten Bondo Christensen, professor, GP and senior researcher Linda Huibers.

 

Task Shifting in Out-of-Hours Primary Care: A PhD Project

The aim of Katrines PhD project is to identify and evaluate a model for task shifting in out-of-hours primary care (OOH-PC). The project consists of three studies: a literature review, a qualitative study based on observations and interviews, and a register-based study.

  • Study 1 is a systematic review. The aim of the study is to evaluate the effect of task shifting from GPs to other health professionals (e.g., nurses, physician assistants and paramedics) in clinic consultations and home visits in OOH primary care by focussing on the content in task shifting contacts and the quality of care provided in the existing models for task shifting.
  • Study 2 is an explorative case study in OOH-PC clinics where task shifting occurs based on observations in clinics and interviews with GPs and nurses working in the clinics. The aim of the study is to get a deeper understanding of how and why the interprofessional collaboration is as it is, including experiences and attitudes of GPs and nurses. The Normalization Process Theory was used as a theoretical framework.
  • Study 3 is a register-based study. The aim of the study is to evaluate the quality of different organisational and collaborative models for OOH-PC clinics on the patients’ care pathway. The main outcomes are resource use.

Use of video in Out-of-Hours Primary Care: A PhD Project

The aim of Mettes PhD project is to evaluate the use of video in out-of-hours primary care (OOH-PC). The project consists of three studies: a register-based study, a questionnaire survey, and an interview study.

  • Study 1 is a register-based study, including all Danish residents contacting OOH-PC in three out of four Danish regions. We used data from the OOH-PC electronic registration system linked to national registers. The aim of the study is to investigate the use of video in telephone contacts to OOH-PC, by studying usage rate, patient- and contact characteristics associated to the use of video, and frequency of triage outcome and follow-up contacts and their association to video use.
  • Study 2 is conducted as a survey among GPs at telephone triage shifts at OOH-PC. We developed a questionnaire, which was integrated into the electronic patient record system and popped-up after a video contact. This technic automatically links background data such as date and time of contact and patient personal identification number to each questionnaire. The aim of the study is to investigate reasons of video use, GP assessed advantages and disadvantages of video use, effect of video on triage outcome, and reason of encounter in video contacts.
  • Study 3 is an interview study with parents to small children, who had a video contact with OOH-PC. The aim of the study aim is to investigate parents’ experiences with video contacts in an OOH-PC setting.
