CONSENT FORM  - Focus groups

Title of Project:  Understanding Barriers within Firms to Implementing Good Work Practices

I confirm that I have read and understand the information sheet for the above study. I have had the opportunity to consider the information, ask questions and have had these answered satisfactorily (required)
I understand that my participation is voluntary and that I am free to withdraw at any time without giving any reason. However, if the focus group has started, I understand that it will not be possible to remove my data from the transcript (required)
I understand that data collected during the study, may be looked at by individuals from The University of Warwick, from regulatory authorities, where it is relevant to my taking part in this study. I give permission for these individuals to have access to my data. (required)
I understand that the entire focus group will be audio or video-recorded. (required)
I agree to selected pseudonymised verbatim quotations to be used in any other reports arising from the research and understand that best endeavours will be made to ensure that data in such publications is anonymised. (required)
The archiving of data has been explained to me. I understand that audio and/or video recordings will be deleted at the end of the project along with my contact details and that the pseudonymised notes will be securely stored for 10 years (required)
I am aware that I need to be considerate of others and not repeat what other focus groups members have said outside of this focus group. I am also aware that IER cannot stop or prevent participants who were in the group from sharing information that should be confidential. (required)
I understand that employers from other regions will be present at the focus group (required)
I agree to take part in the above study. (required)

Personal data will not be passed on to any third parties and will be kept securely in a password-protected folder at the University of Warwick for the purposes of this research.
