I understand that my participation is voluntary and that I am free to withdraw at any time without giving any reason, without my legal rights being affected. (required)
I understand that data collected during the study may be looked at by individuals from The University of Warwick, from regulatory authorities, where it is relevant to my taking part in this study. I give permission for these individuals to have access to my data (required)
I agree to allow the interview to be audio-recorded for the purposes of this research (required)
I agree to allow anonymised verbatim quotes to be used for the purposes of reporting. (required)

Privacy notice

Personal data will not be passed on to any third parties and will be kept securely in a password-protected folder at the University of Warwick for the purposes of the research. We will contact you to arrange a telephone interview to ask you about your own experiences of transitioning out of the Services (whether you are a SNCO or spouse/partner of a SNCO).
