1. I confirm that I have read and understand the information sheet (version 1, 28/03/22) for the above study. I have had the opportunity to consider the information, ask questions and have had these answered satisfactorily. (required)
2. I understand that my participation is voluntary and that I am free to withdraw at any time without giving any reason. (required)
3. I understand that data collected during the study, may be looked at by individuals from The University of Warwick, from regulatory authorities, where it is relevant to my taking part in this study. I give permission for these individuals to have access to my data (required)
4. I agree to allow the interview to be audio or video recorded for the purposes of this research only. If not, you can still take part in the study if you tick the following item. (required)
5. I agree to allow the interviewer to take notes on this interview for the purposes of this research only. (required)
6. I agree to pseudonymised verbatim quotes from the interview to be used in any other reports or publications arising from the research, and understand that best endeavours will be made to ensure that data in such publications is anonymised. (required)

Privacy notice

Research data will be pseudonymised as quickly as possible after data collection. This means all direct and indirect identifiers will be removed from the research data and will be replaced with a participant number. The key to identification will be stored separately and securely to the research data to safeguard your identity. 

Your rights to access, change or move your information are limited, as we need to manage your information in specific ways in order for the research to be reliable and accurate. The University of Warwick has in place policies and procedures to keep your data safe.  

The research data may also be used for future research, including impact activities following review and approval by an independent Research Ethics Committee and subject to your consent at the outset of this research project.
