I understand that my participation is voluntary and that I am free to withdraw at any time without giving any reason (required)
I understand that data collected during the study, may be looked at by individuals from The University of Warwick, from regulatory authorities, where it is relevant to my taking part in this study. I give permission for these individuals to have access to my data. (required)
I agree to allow the interview to be audio recorded for the purposes of this research only. If not, you can still take part in the study if you tick the following item. (required)
I agree to allow the interviewer to take notes on this interview for the purposes of this research only (required)
I agree to pseudonymised verbatim quotes from the interview to be used in any other reports or publications arising from the research. (required)
I understand that audio recordings will be deleted after transcripts have been quality assured and that the pseudonymised transcripts / interview notes will be securely stored for at least 10 years. (required)
